Insurance and Reduced Fee Plans We Participate With:
If you have dental benefits from any of the insurances listed below in which our office participates, you will be responsible for a co-payment percentage or a reduced fee (if you have a reduced fee plan) at the day of the visit. Please remember to bring your insurance card with you to our office. We will submit your claim to the insurance company on the date of service.
If you have dental insurance in which we do not participate, we can develop a fee that may accommodate your needs to be paid at the day of the visit.
If you have dental insurance in which we do not participate, we will assist you with submitting claims for reimbursement from any insurance company that allows you to choose your own doctor. However, please keep in mind that our arrangements are with you and not with your insurance company. Therefore, regardless of any insurance payment or reimbursement, the entire fee for your treatment is ultimately your responsibility.
