Your First Visit
Thank you for CHOOSING our office for your oral surgery care. In order for us to provide and treat you with the best care possible, please provide us with the following information at your first visit:
Your yellow referral card from your dentist and any x-rays if applicable.
If you have medical or dental insurance, bring the necessary insurance cards or forms. This will save time and allow us to help you process any claims.
If you are a college student, please bring proof of enrollment.
A list of medications you are presently taking. Most Importantly if you are taking any blood thinners (ie. coumadin, plavix, or aspirin)
If you are taking Coumadin, please provide us with your cardiologist's telephone number or the Doctor that controls your Coumadin intake, in order for us to get medical clearance for your Oral Surgery appointment. In addition you must provide us with your most RECENT INR levels.
If you are currently taking any medications for osteoprosis or bone cancer, please bring that information.
If you need to take any premedication prior to having any dental procedures, please inform us when you are making your appointment.
